Gleevec

Gleevec (imatinib mesylate) is a biological therapy drug often used to treat symptoms associated with chronic myeloid leukemia (CML), such as night sweats, fever, loss of appetite, and weight loss. CML is an uncommon type of cancer that affects bone marrow, which then affects the blood because the bone marrow makes blood cells.

Gleevec is also sometimes used to treat gastrointestinal stromal tumor (GIST), which is a rare form of stomach cancer. Typically, removing the tumor is the best (and preferred) course of treatment for GIST, but it’s not always possible. When the tumor is too large or surgery is too risky, Gleevec may be used to shrink the tumor until it’s small enough (or the risk is reduced enough) to remove surgically.

Imatinib mesylate works as a tyrosine kinase inhibitor (TKI), which — as the name suggests — blocks a chemical called tyrosine kinases. TKIs stop tyrosine kinases from sending growth signals to cells, which prevents cancer cells from growing or multiplying.

Brand name Gleevec is increasingly difficult to find in the United States. This is relatively common among well-established drugs with generic equivalents, in part because the customer typically wants to pay a lower price if given the option, but also because the insurance company also wants to pay a lower price.

Lysodren

Lysodren (mitotane) is a prescription drug used to treat adrenal cortical cancer (ACC), a rare disease in which cancer develops on the outer layer (cortex) of the adrenal gland(s).

The adrenal glands are small glands located just above each kidney (one per side). They produce a few different hormones, including adrenaline and two steroids: aldosterone and cortisol.

Tumors that can develop on the adrenal glands can be either cancerous or benign, but adrenal cancer is nothing to be trifled with. Because they produce critical hormones for the body that literally make other systems function properly, a poorly functioning adrenal gland (or two) can have severe health consequences.

Surgery is really the preferred treatment method, but it’s not always possible. In those cases, Lysodren may be prescribed to help stop the spread of cancer.

While experts are still unclear as to why Lysodren works, the process is done by stopping the adrenal glands from producing adrenocorticoid hormones (steroids) that cancer cells feed on. By cutting off their “food” supply, they might starve and die, which could decrease the number of cancer cells in the body.

Lysodren is often difficult to find at a pharmacy in the United States. In many circumstances, it’s specially ordered through a physician, or is purchased through a medical facility. This allows them to charge high prices because they’re restricting the supply, but to be fair, Lysodren won’t have a naturally high demand because ACC is so rare. And as we know, drugs for rare diseases are often very expensive.

Rheumatrex (Methotrexate)

Rheumatrex (methotrexate) is a popular cancer drug because it can be used in so many situations. Most commonly, Rheumatrex is used for the treatment of acute lymphoblastic leukemia, non-Hodgkin's lymphoma, bone (and soft tissue) cancers, and a number of solid tumors, particularly those associated with breast, lung, head, neck, bladder, cervical, ovarian, and testicular cancers.

Rheumatrex is not typically the first line of defense in these situations. Most often, a different prescription has been attempted, but with disappointing results.

This type of drug interrupts the growth of certain cells in the body, particularly those that multiply quickly, such as cancer cells. However, Rheumatrex doesn’t just stop at cancer cells — it also interrupts bone marrow and skin cell reproduction, which can cause unwanted side effects.

Because this cancer drug has been on the market for so long, it can be difficult to find the brand name anywhere. Fortunately, it’s relatively affordable.

In the U.S., you can get 30 tablets generic methotrexate (2.5 mg) for about $30.00 ($1.00 per pill).

The way antimetabolites work is fascinating because it’s so targeted. When people think of chemotherapy, they often think of killing cells en masse and indiscriminately as the drugs try to kill the cancer cells. While antimetabolites do kill cells, they only target cells that are dividing.

Inside the body, they mimic substances that cells need in order to grow and multiply. The dividing cells get the antimetabolite instead of what they need, which blocks the replication of their DNA. This can effectively stop cancer cell growth. For this reason, antimetabolites are often used to shrink tumors, and as part of an overall course of treatment for various types of leukemia.

Purinethol in particular is often used to treat acute lymphocytic leukemia, which is most common in children, but can also occur in adults. It’s also used at times to treat lymphoblastic lymphoma, as well as Crohn's disease and ulcerative colitis.

Because Purinethol has been on the market for many years, there is a generic readily available. In fact, that’s what you’ll most likely find through American pharmacies. Generic mercaptopurine will cost about $42.00 for a one-month supply ($1.40 per pill).

By inhibiting androgen receptors, Xtandi decreases the frequency at which testosterone can bind to said receptors. This may decrease the size of a tumor, but is not a cure for prostate cancer.

Typically, medications like Xtandi are not the first cancer drug prescribed but are instead used when other treatments aimed at lowering testosterone levels are not effective. There are many reasons for this methodology, but one of them is certainly a matter of cost and availability.

Xtandi is very expensive and does not currently have a generic version available. It’s considered a “specialty drug,” which severely restricts its accessibility. In the U.S., you may not even be able to purchase it on your own — it may need to be facilitated by a doctor’s office or hospital system.
